The main objective of the project is to improve activities in the field of mental health by testing the model of support for people with mental disorders (OzZP). The model will be tested in three districts of Warsaw – Wola, Targówka and Prague Północ. The project will be implemented in three phases: 1. The preparatory phase will include, among others, the employment of project staff, rental and equipment of premises for Advisors, renovation and equipment of apartments protected for OzZP.2. the testing phase will cover the implementation of individual stages of the model.3. The evaluation phase will be conducted throughout the testing phase and will result in a modified model taking into account the test results. The target group of the project is 1050 OzZP, residents of Warsaw and their surroundings, users are all entities involved in the OzZP support process. The project will be implemented in accordance with the concept of empowerment. OzZP and their surroundings are actors involved in the support process, they co-decide on forms and working methods, their resources and potential are used. They will be active participants in the development of Individual Mr. Recovery and Support. P is publicly available, in the case of qualified persons with disabilities with special functional needs, the possibility of triggering a rational improvement mechanism (e.g. acoustic access, easy-to-language interpreter, sign language translator, interpreter-guide, guide for people with weak visions, etc.) The assumed main effects are: — increase of subjectively felt quality of life,- improve the functioning of individual social networks, – increase the level of social functioning,- optimisation of the process of recovery, – increased sense of strengthening, – improvement of the quality and comprehensiveness of the offered support,- development of forms and principles of cooperation between entities supporting OzZP.
