The objective is to implement quality and organisational measures in the health system to facilitate access to affordable, sustainable and high-quality health services. The project will enable the creation and implementation of MPZ for 30 disease groups in favour of OZ in 16 voivodships and the country. 30 models will be developed and based on them MPZ. Each MPZ with the scope defined in the Disease Group Regulation will refer to 3 modules: LZ, AOS, POZ. A list of indicators based on standards will be created for each module.They will be adjusted for expertise in the functioning of SOZ in Poland. This knowledge will be based on consultation with practitioners and experts. The project activities will complement the work of national health policy. Models for 15 diseases (A) in the field of LZ will be developed by 06.2016. By 12.2016, further models for AOS and GP and 15 MPZ for LZ (B) will be developed. By 12.2017, AOS and GP models for 15 disease groups (B) will be developed. The developed models will allow to present at the end of the year 2016 16 regional and 1 national MPZ in the field of LZ for 30 groups of diseases and AOS/POZ for 15 diseases in group A. 16 regional and 1 nationwide MPZ in the field of AOS/POZ for 15 diseases in Group B will be developed by 12.2017. The project provides for a series of workshops, consultations and conferences with SOZ Stakeholders to discuss the assumptions of models and MPZs. They respond to the need to build capacity in the social partners, drawing on the usefulness of the MPCs.
