The overall objective of the project is to improve the quality of life of people with disabilities from the Municipality of Smolyan and their families by providing basic social and health services in the community or in a home environment. The target group of the project is:1.People with disabilities and their families. 2.The target group, who use services under the New Alternatives procedure, to whom a new needs assessment will be prepared after the completion of the project in March 2016. The planned duration of the project is 22 months, of which 18 months will be provided services in a home environment of a total of 80 staff assigned to the posts: 30 personal assistants, 30 social assistants, 20 domestic assistants, a nurse, a rehabilitator and a psychologist. A leading principle in the implementation of the project will be placing in the “center” the needs and needs of each individual.The main activities that will be implemented are:1.Management and accountability2.Information and publicity3.Applying, evaluation and ranking of applicants for use of hourly services from a Service Unit in a home environment4.Applying, assessment and ranking of staff for the provision of services in the home environment5.Providing access to integrated services according to the specific needs of disadvantaged people based on individual social assessment provided by the Service Unit in a home environment, including supporting and integrated cross-sectoral services6.Training of staff to provide services7.Consultation and support of staff providing social services8.Providing support to service users, according to their individual need9.Purchase of equipment necessary for the quality provision of the services and implementation of the projectWith the realisation of the project we aim to cover 150 people from the target group, who will receive the appropriate support for them for independent living and decent life.The total cost of the project amounts to 500 000 BGN
