The project proposal will be implemented by providing support under Priority Axis No. 2 of OPHRD “Reducing poverty and promoting social inclusion” and is in line with basic strategic documents aimed at creating conditions for inclusion of persons with disabilities, including persons with severe disabilities and their families and with the National Strategy for Long-term Care and the National Health Strategy 2014, and the Centre is established according to the Community Support Services Map of the Action Plan 2018-2021 for the implementation of the strategy.The main objective of the project proposal is to provide quality care for persons with disabilities and to be an innovative model of their social service, including the establishment of an innovative model of social service, including the establishment of a functional centre for families with disabilities. Target groups are:-Persons with disabilities, including severe multiple disabilities at risk and their families;- Persons with disabilities, including severe multiple disabilities, using resident services and their families; Through the implementation of the activities foreseen in the project will create conditions and conditions aimed at improving access to social services and services of the target group. The project establishes a Day Care Centre for persons with disabilities and their families, including with severe multiple disabilities — Asenovgrad, which will provide daily, half-day and hourly services — rehabilitation, therapy, counselling, information and support to 35 people in need. It is planned to carry out repairs and equipment of a suitable building, as well as the purchase of special transport for the provision of mobile services, other supporting activities are foreseen, according to the specifics and needs of the target group.
