The project supports the realisation of the economic, social, health and cultural rights of young people entitled to support services under the Act on Integration. The project also responds to the presentations of the post-care aftercare team (2019), utilises the results of the post-care modelling work (Yliruka et al. 2020) and taking into account the concerns about the system’s silo and the ethos and principles set out in the report of Archbishop Kari Mäkinen’s working group (2020).The project aims at changing operating methods at three levels, which coordinate the objectives and examine the whole systemically: national, local and individual. Preparation for structural change in aftercare requires multi-level development work.1) The project strengthens the realisation of the rights and equality of young people in the target group. It aims to promote the prevention of inequalities and exclusion, as well as the promotion of equality and inclusion. The project supports equality among the target group, prevents and reduces inequalities, and supports the realisation of equality of opportunity and inclusion also in the context of the pandemic. Support the realisation of the equality and rights of young people at all levels: aftercare social work in the development of customer plans, local co-development of services, and national cooperation and influencing.Product: the main issues that hamper the achievement of equality among the target group have been identified and reported at all levels, and proposals for the necessary actions in the areas of health, social services, education, employment, housing, social and family relations and support for independence have been identified.2) The project will compile a knowledge base to support the development work. Data collection on municipalities’ services and young people’s needs will be carried out. The project will conduct surveys on the organisation of child protection after-care and support measures under the Act on Integration. The results are communicated through multi-channel communication to municipalities and partners as well as young people. The results of these studies will be presented and the conclusions will be discussed with national and regional actors. The joint processing of collected information and conclusions will bring about changes in the understanding of professionals and young people, and the results will influence local practices by developing them. As the awareness of the target group grows, young people are more sensitive to the need for support measures that meet their needs, and the needs become visible in municipalities. 3) The project will develop integrated service and operating models to meet the needs of the target group in cooperation between different sectors, between the public and third sectors, and to strengthen coordination and the participation of young people in the development. Support co-operation and peer-to-peer development between sub-projects by organising workshops to reflect lessons learned and work on modelling. Result: New cooperation networks have been created locally, service integration and youth inclusion models have been piloted, information on good practices has been learned and learned from them, structures for cooperation have been created, customer planning modelled, a strengthened dialog culture, increased customer confidence in the service machinery. The project will strengthen multidisciplinary collaboration and create new networks and operating models of development, learning and influencing in national and local activities, as well as consolidating their activities in support of the structural change in aftercare. Measures: Organise workshops and development forums together with young people. Listen to and gather proposals and initiatives from both young and professionals in the course of the process.Outcome: Multidisciplinary service needs assessment and consultation of young people in service design, involving young people, is becoming more common. New types of cooperation networks serve as forums for development, learning and engagement at national and local level. Peer-to-peer networks of experience experts work, young people are involved in the development, their proposals have been heard, they are able to ask and demand their rights and assistance. Create co-creation platforms that can contribute to the renewal of practices. Achieve effectiveness by taking messages to different administrative municipalities in pilot municipalities and promote the roots of developed models.The gender perspective will be mainstreamed into the project’s activities. In practice, this means taking into account specificity between men and women and other sexes, as well as seeking to alleviate segregation. The project promotes social equality by strengthening the opportunities for inclusion of young people who experience discrimination and disadvantage also in the context of the pandemic.
