The present national project represents the first step of synergistic support for the transition from institutional to community-based care in the Slovak Republic (hereinafter referred to as the Slovak Republic) resulting from the international commitments of the Slovak Republic and the implementation of the Strategy for deinstitutionalisation of the system of social services and alternative care in the Slovak Republic The main objective of the project is preparation, creation and systematic methodological support of transformation teams in the creation of transformation plans of specific social services facilities (hereinafter referred to as “the ZSS”) involved in the process of transition from institutional to community-based care and the start of transformation processes in communities where these facilities will be located. The result of this support will be the creation of transformation plans for specific RCGs, which are essential to ensure synergies with the Integrated Regional Operational Programme (IROP). The preparation of the ZSS involved in the project will be carried out mainly through the provision of continuous methodological support, education of relevant persons and entities, dissemination activities supporting the process of deinstitutionalisation (hereinafter referred to as “DI”) and community relations, supervision and information activities on the deinstitutionalisation process. The national project consists of 7 sub-activities – the activities of the Project Steering Committee, the establishment and operation of the methodological and coordination team, the selection of the devices involved, the assessment of the readiness of the devices for the DI process, education for the participating ZSS, their founders and other relevant actors, consultation of the preparation of transformation plans, dissemination activities of the transition process from institutional to community-based care. The project will be implemented in partnership IA MPSVR SR and three partner organisations – RPSP, CEDA, SUPZ. Each partner involved will be responsible for basic areas of support in the deinstitutionalisation process – social services, changes in the physical environment, employment and activation.
