The main objective of the planned actions relates to the achievement of the specific objective of the ROP WZ 2014-2020, i.e. to increase the availability of social services, in particular environmental, care and support services for people at risk of poverty and/or social exclusion. The main objective of the project is to ensure the availability of quality care services for 700 people in the West Pomeranian Voivodeship by March 2023. The main tasks include: creation of a Regional Advisory Point – remote counselling, on-site and on-site counselling, training, self-help groups – for at least 400 people (actual caregivers of dependent persons and persons providing social services in the local community and candidates for these functions); removal of architectural barriers in at least 5 places of residence; launching tasks in the field of telecare (IT platform, calling wristbands) setting up at least 5 respite apartments (adaptation, equipment) in accordance with the principle of deinstitutionalisation – a minimum of 8 seats for at least 16 people at risk of social exclusion; organisation of neighbourhood care services for at least 180 people for at least 700 dependents (persons at risk of poverty and/or social exclusion K/M); setting up 2 rehabilitation equipment rentals; creation of 21 volunteering points (in each district of WZ); conducting community activation campaigns and activities to recruit neighbourhood carers; actions to prevent and counter COVID-19. The project is addressed directly to the following group of recipients: 716 (321K/395M) people at risk of poverty and/or social exclusion; 400 people (350K/50M) actual caregivers and social service providers in the local community and candidates for these functions.
