The project is aimed at 400 inhabitants of the revitalised area (the centre of the city together with neighbouring districts, which live in approx. 22320 persons) at risk of poverty or social exclusion and their families, and in particular: seniors, OZN, families characterised by helplessness in care matters and children and young people coming from such families. The planned actions will cover 4 main initiatives: —development of care services and specialised care services. in the place of residence, including the organisation of meals – development and dissemination of telecare services together with the necessary accompanying infrastructure and the creation of a Local Coordinating Point providing personalised support to seniors and persons with disabilities and stimulating their activity (AON, AOON, animator) – creation and operation of a psychological support facility (including street service). The aim of the project is to create new, sustainable places to provide social services and to expand and improve the existing support system, using infrastructure created in the revitalisation process. The actions undertaken will contribute to raising the standard of living of seniors and OZN, extended or increased their independence or will be possible to function. with external support but in the place of residence, and the need to stay in facilities providing/services will be postponed over time, possibly completely offset. On the other hand, actions aimed at families who are not fully capable of education will contribute to raising the level of parental education, the process of social exclusion of children and young people will be hampered, which will reduce the number of children sent to foster care. Actions implemented in the framework of the project will continue in the context of sustainability.
