The aim of the project is to promote social inclusion, facilitating access to community services, by increasing the scope and quality of the services provided in the local community. Self-reliant support will be provided in the form of care services in the place of residence, rehabilitation, assistants, counseling, therapy and courses. The social services will be supplemented by the use of modern information and communication technologies (telecare) and the creation of medical and rehabilitation equipment rental. The project also includes the support of actual carers in the form of counselling and workshops increasing their knowledge and skills in the field of assistance for dependants and service providers. The project supports the process of disinstitutionalisation of social services, by enabling an independent person to function independently on a Wednesday preventing isolation from the family and the local community. Project in line with the concept of universal design and EU horizontal policies: — equal opportunities for K/M and non-discrimination (public project, gender sensitive language, taking into account GD intellectual level, activities enabling people to function independently in the environment) – sustainable development (actions to reduce poverty and social exclusion, increasing participation in society, budget based on market prices reflecting real costs, use of green paper and office articles in project management) – information society(use of modern TIK project, etc.) Project compatible with Płocki progr. Revitalisation(C.1.3Creation of conditions for the activation and integration of residents and the development of entrepreneurs., C.2.3 Activation and integration of people from passive and marginalised groups, C.3.3Creating conditions for social reintegration and incl.), covers revitalised areas and contributes to counteracting negative social phenomena in these areas.
