The project has two main areas. One is to make the coordinated individual plan available as a resident service (e-service) to the individual and close relatives to satisfy the need for participation and co-creation in their own care and care. The other area is to provide health and social care personnel with modern efficient operational systems and communication services (e-services) that entail the establishment, execution and monitoring of the activities that can be housed in an individual plan coordinated and can be carried out as efficiently as possible. The project’s goal is that the coordinated individual plan should be available electronically for patients and relatives as a resident service via 1177/MVK (My Care Contacts) which provides increased participation and a better overview of the care processes and contributes to increased person-centration and increased patient safety.
